PRASE: Patient Reporting and Action for a Safe Environment

PRASE involves collection of patient feedback around the safety of care. A questionnaire called the ‘Patient Measure of Safety’ (PMOS) is used to collect patients' feedback about their care and it is based on scientific evidence (Yorkshire Contributory Factors Framework) about the factors that are important for safety (e.g. communication, resources, ward environment)

An app and reporting website supports the collection of feedback and generates reports for staff. Once feedback is collected from approximately 20 patients on a ward, a feedback report is provided for multi-disciplinary ward teams, who then consider the feedback in an action planning meeting.

Teams then make changes to improve the quality and safety of their care.
